How to Find Your Vaccine Records on MIIS?
Massachusetts Immunization Information System (MIIS) allows citizens to access their vaccination records. Citizens can request, view, and print their vaccination records and COVID-19 SMART Health Card online from the MIIS portal.

- Visit the official web portal https://myvaxrecords.mass.gov/pages/Registration.

- Create a 4-digit PIN to access your records.
